A care pathway for pharmacies that help patients stop smoking. Sign-up, five follow-up moments and personal e-mails run by themselves. If someone is about to drop out, the pharmacist gets a nudge straight away, and an overview shows how each patient is doing.



Challenge
Without a system, stop-smoking support is done by hand: no fixed follow-up and no signal when someone is about to drop out. Everything depends on the pharmacist's time and attention.
What it does

The patient signs up and the five follow-up moments are scheduled straight away
- 02Follow up
At each moment the patient gets a personal e-mail, tailored to their smoking history and medication
- 03Check in
With their own link, without logging in, the patient says each day how it is going
- 04Flag
If someone stops responding or gets worse, the pharmacist gets a nudge straight away
Result
- Follow-up runs by itself; the pharmacist steps in when nudged
- Five personal follow-up e-mails per patient
- A risk score that warns straight away when someone is about to drop out
Technology
An end-to-end automation system: the patient signs up through an intake form, n8n automatically schedules 5 follow-up moments (day 7, 14, 30, 60, 90), OpenAI writes personalised e-mails, and the patient gets a personal app link without a login. At a high drop-out risk, the system sends the pharmacist an alert straight away. Patient app on Cloudflare Pages with a UUID link without a login; the risk score counts non-response and mood and warns above 70; OpenAI gpt-4o-mini writes the e-mails.
